Q725. Name missing from birth certificate how can I apply for Indian passport
Question: Dear Sir I have birth certificate but I have not name in on that shall I apply passport, without name birth certificate. Please guide me chef muthu [February 09, 2016]
Answer: If the Birth Certificate doesn’t contain the name of child, a declaration on plain paper signed by parents, is required to be submitted specifying the name of the child. Hope this helps. [ As per Document Number 2 of Table 3 in Section D of Passport Seva]
