Even though your wife is currently not a Pakistani citizen currently, the fact that her place of birth is Pakistan and she may have been a Pakistani citizen before acquiring foreign citizenship would disqualify her from being eligible of PIO card. The rule for eligibility for a PIO card, disqualifies anyone who was at any time a citizen of Afghanistan, Bhutan, China, Nepal, Pakistan and Sri Lanka. You could of course confirm with the Indian consulate.
