Now how can she apply without birth certificate. is it possible if she get affidavit from govt lawyer for her birth? I need help from you please reply with best way to get passport quick. Thanks. Rajiv kumar Answer: Many people have the same problem you are having where the date of birth on municipal birth record and school certificate does not match. However, my understanding is that only those applicants for a fresh passport who were born before 26th of January, 1989 can use their School certificates as proof of date of birth. They have the option of providing ONE of the following documents: Birth certificate Or School leaving certificate Or Affidavit [Affidavit sworn before a Magistrate/Notary stating date/place of birth as per the specimen in Annexure 'A' by illiterate or semi-illiterate applicants (Less than 5th class)] As your wife is born after January 26, 1989, she does not have the options shown above. Any person born after January 26, 1989 must provide a birth certificate issued by a authorized office. Since your wife was born in 1991, she is required to submit birth certificate issued by Municipal Authority. Assuming that the birth certificate date on the document issued by the Municipality is correct, she can simply submit that document.
