Question: Hello, I have been trying to complete an application for my father's replacement OCI. His passport and OCI card were stolen. We have a police report that outlines this. I have tried to complete the form online, but it asks for the passport number that was associated with the OCI card (I do not have this, I only have the new passport number). It also asks for reference information from the last OCI card, I also no longer have this. My father's name is ************ He was born in New Delhi, India. His birthdate is 11/13/1946. Please can you provide us with details on how to proceed? My father will need to return to India in the next month. I appreciate any guidance you can provide. Kindest regards, Answer: You can get your father’s stolen OCI Card replaced with a new one but NOT by going directly to the OCI Miscellaneous Services web page. As you have already found out you cannot apply for replacement online without providing information about your previous OCI card, which you don’t have. To proceed through OCI Miscellaneous Services applicants MUST provide 1. Passport Number * (as in OCI card) 2. Any one of the following fields: U-Visa No # Or OCI Number # Or OCI File No # 3. Date of Birth Or Mother’s Name If any of the above three fields are not known, then access to the Form is denied.How to apply for replacement of lost/stolen OCI card in USA if you don’t have your lost OCI

Apply through the outsourced company. In USA it is CKGS. 1. Go to the CKGS website https://www.in.ckgs.us/oci/ 2. After filling Applicant details you will be directed to the Form for applying for OCI Card. Under ‘Choose your Category’ Select OCI Re-issue for lost OCI Card [under category #3] 3. Complete the Form as required. Those who do not have a copy of the lost OCI card and do not remember any of the particulars of their lost card may have to provide a Notarized Affidavit for the lost OCI Card. This is in addition of the required documents for OCI. Hope this helps.
