Q.846 School Certificate not available | Any Options to get Indian Passport
Question: Dear Sir/Madam
My wife was born on 23/02/1976 but she has lost his school certificate and as when she passed high school it was
under M.P. Board now it has come under Chattisgarh Board so we are facing difficulties in issuing a fresh school
certificate. so what are the options we have for Birth Certificate for passport.

Answer: The Chhattisgarh Board when they took over would have taken over the previous records and you should
apply to them for your wife’s school certificate by providing dates etc.
Here is a situation where education seems to hurt! If a person did not go to high school, was uneducated and had no
birth certificate, they could simple provide a one page affidavit in lieu of their birth certificate to get a passport.

Go to the Passport Seva site: http://passportindia.gov.in/AppOnlineProject/online/annexureAffidavit

On the line that says: Affidavit of Date of Birth to be submitted by illiterate Applicants (Annexure "A") click on
‘Annexure A’

You will see the document reproduced below: This one page affidavit would be fine if a person is uneducated,
however since your wife is educated, she must provide her school certificate for verification purposes.
